Rhinoplasty: what is it?

Rhinoplasty is the most commonly performed facial surgery in the world and its aim is to improve aesthetics, but never forgetting the functional side.

In fact, the nose is a unique structure that cannot be seen as a central aesthetic element of the face without considering that it is the first organ of the respiratory system.

The surgeon must therefore master the anatomy and surgical concepts of the nose as a whole and not in sections.

At the first appointment, a candidate undergoes a complete assessment of the entire nasal structure, from the outer pyramid and its relationship with the rest of the facial anatomy, to the endo-nasal structures such as the nasal septum and turbinates and the peri-nasal sinuses.

When nasosinusal pathology is diagnosed, it can be corrected during the same surgical procedure.

The desired result is analysed in terms of the overall face, seeking a harmonious nose that fits into the individual's particular context without the stigma of an "operated nose".

In fact, computerised simulations and pre- and post-operative photographs of other similar cases are used as tools to help with this programming and discussion. To this end, photographs are always taken before and after surgery.

This surgery has undergone advances in concepts and techniques over the last few decades, just like facial surgery in general.

In most cases, no external scar is used. When it is essential, it is placed in the columella (the central portion separating the two nostrils) where visibility is reduced and healing is favourable.

Facial haematomas, mainly peri-ocular (around the eyes), are only present when osteotomies are performed (fractures of the nasal bones for repositioning). However, despite the apparent aggressiveness, the surgery is practically painless.

Septoplasty

Septoplasty is surgery that corrects deviations in the central structure of the nose, the nasal septum.
This intervention is carried out when there are functional problems caused by obstruction of the nasal cavities, resulting in nasal respiratory insufficiency and/or sinusitis, and when there are aesthetic problems if the deviation of the nasal septum affects the positioning of the nasal pyramid. The nasal septum is an osteo-cartilaginous structure covered by mucosa. To attach the skeletal structure, a vertical incision is required in the mucosa of the nasal vestibule (at the entrance to the nose), which will not be visible. The surgery is usually carried out on an outpatient basis, so it is only necessary to stay in the clinic for the first 4 hours after surgery.
The post-operative period is generally painless. Post-operative discomfort is mainly due to the nasal tamponade (when necessary), which remains in place for 1 or 2 days. It's worth noting that the nasal packing is a perfectly painless moment, as we use a personalised, delicate, plasticised packing, which allows for atraumatic sliding.
Post-operative recovery takes 5 to 7 days.
